I find that if I get hit in sparring I tend not to notice it until afterwards. The amount of times I've woken up next morning to discover some hitherto unnoticed bruise which is throbbing away.
Being hit when unprepared is an odd experience. I've been tapped on the nose when day dreaming durung tegumi drills. It didn't hurt but the shock was pretty amazing.

I have been hit loads of time, in and out of the ring. I have just had what I hope is the final operation (no training for another 3 months, bah humbug) to correct something which happened a long time ago. Some idiot came round a corner and landed one on, it got very messy, and I decided to bleed all over them. This is where adrenalin is great; you tend not to notice being hit until the day after.

Been hit plenty of times, and hurt myself hitting others plenty of times. In many cases the incidents were not 'outright' assaults, but incidental or accidental. But, many more were deliberate, including having my thigh struck by an elbow, causing it to stiffen and stay stiff for months, having my nose smacked countless times (three operations, including repair of deviated septum, removal of bone fragments, etc), bruised gums (from mouth impacts), chipped teeth, torn muscle/ligament/tendons, nasty inflammations, dislocation of wrist, both knees, ankle, and shoulder, etc. Things happen. Deal with it, heal as best you can, and move on.
Adrenalin does cover up a lot of the injuries you obtain during a confrontation, but many simply do not present themselves until later... after the swelling takes place. Also, adrenalin actually exacerbates back injuries or spazzing muscles. If you have either of these, you'll find adrenalin to be 'really annoying,' as it makes either of those hurt all the more while your adrenalin is pumping. Worse is that such pain, while filled with adrenalin, undermines your confidence.
Noses are a fun thing. As previously indicated, i've been tagged plenty of times there. For quite a long time my nose was broken and i didn't know it, so everytime someone 'bumped' it, i was in excruciating pain. I thought that was 'normal,' and thus got used to ignoring it. It was not until months after my first operation, when i bumped my nose on a window pane (don't ask) that i was shocked to find... "wow! so this is what a real nose feels like." No pain.
